Let St. Luke’s Adult Day Health Care Program Help

Registrants in our Adult Day Health Care Program are offered a variety of services that are individualized to meet specific needs. Personal care services range from:

  • Health and medical care, including case management
  • Health assessment
  • Physical rehabilitation therapy, occupational therapy, speech therapy
  • Therapeutic diets, nutritional education and more

In addition to the full array of medical services available, registrants can also involve themselves in the many varied activity programs that take place at St. Luke throughout the day. These activities and outings to area attractions are designed to keep registrants engaged and involved while they receive quality health care in a safe and supportive environment.

How to Access St. Luke Adult Day Health Care

Adult Day Health Care at St. Luke serves all of Oswego County. Because we are centrally located in the city of Oswego and easily accessible, local medical transport agencies, family or friends can bring registrants to us or we can arrange to have your loved one picked up and returned home. Adult Day Health Care is a Medicaid eligible program.
