To become a St. Francis Commons senior resident, please complete our application package, which includes:
- Application (printable PDF)
- Copies of all insurance cards, driver’s license, or other photo ID
- Advanced Directives (Power of Attorney, Health Care Proxy, DNR, etc) as applicable
