Adult Day Health Care
Our Adult Day Health Care is a daily health monitoring and activities program that’s designed to promote independence—and delay institutionalization—of disabled adults. We opened the first “medical model” Adult Day Health Care program back in 1993, so you can feel confident knowing that we are the most experienced provider in our area.
Located at St. Luke Health Services in Oswego, Adult Day Health Care provides a supportive and friendly atmosphere for adults age 18 or older that require ongoing rehabilitation and medical services. Open Monday through Friday, we offer a variety of attendance options. Registrants can attend up to five days per week, or as few as one day per week.
Registrants are offered a variety of services that are tailored to fit each adult’s specific needs. Personal care services range from health and medical care to:
- Case management
- Health assessment
- Physical rehabilitation therapy
- Speech therapy
- Therapeutic diets
- Nutritional education, and more!
In addition to the full array of medical services available, there are a multitude of activity programs and outings at St. Luke that are available throughout the day for ADHC registrants. These activities keep registrants engaged and involved while they receive the health care they need.
St. Luke ADHC serves all of Oswego County. Our central location in the City of Oswego makes us easily accessible. Local medical transport agencies, family, or friends may bring registrants to us, or we can arrange for pick-up and return home.
Adult Day Health Care is a Medicaid eligible program.
