We know that most people wish to receive health care and services right in their own homes, and we can help. St. Luke Home Health Services is a New York State Department of Health licensed home care agency serving Oswego, Jefferson and Onondaga counties.
We provide personalized services to meet your individualized needs in your home setting, all designed to help you maintain or achieve maximum independence.
Whether you suffer from a chronic illness or are recovering from a recent surgery or other medical event, we provide the flexibility to assist you as changes occur in your health.

How do I arrange for Home Health Services?
With a simple phone call (315) 349-0798, we will schedule a no obligation nursing consultation will assess your health and personal care needs, we will need a “release of information” agreement to contact your doctor. Before services begin, you will receive an orientation to our Home Care Agency services. Our nursing staff will assess each client’s needs and put into place an individual care plan to address those needs.
Our staff consist of Registered Nurses, Licensed Practical Nurses, Home Health Aides, Personal Care Aides, and Homemakers. St. Luke Home Health Services staff participate in Quality Assurance activities to ensure a high level of service, as client care is our top priority.
Users of our Home Health Services pay privately or through their insurance (depending on the private insurance coverage). We bill on a monthly basis.
