Become a Resident
To become a St. Luke Health Services resident, please complete our application package, which includes:
- Application (printable PDF)
- Copies of all insurance cards and Driver’s License or other photo ID
- Advanced Directives (Power of Attorney, Health Care Proxy, DNR, etc) as applicable
- *Patient Review Instrument (PRI) and screen
*The PRI is a NYS mandated medical assessment tool that determines long-term care eligibility and assesses level of care required. It is completed by a certified community PRI service or hospital (if applicant is currently in the hospital.)

St Luke will request medical information from the primary care physician or hospital (if applicant is currently in the hospital,) review the application package, and notify the applicant of their status.
